Model Release & Consent Form (Template)

Creators must hold written consent and a copy of a government-issued photo ID for every person who appears in their content, as required by the Content Provider Agreement. This template satisfies those requirements. Print it or copy it into a document, have each person sign it before you publish, and keep the signed copy together with a copy of their ID for at least seven years. You may use your own form if it covers the same points.

Do not upload signed releases or ID copies to CosplayXclusive unless we ask for them. Keep them privately and securely.

I, the Model, confirm and agree that:

  1. I am at least 18 years of age. I have shown the Creator a valid government-issued photo ID and permit them to keep a copy for verification purposes.
  2. I consent to being depicted in the content described above, including in any nudity or sexual content we have agreed to in advance.
  3. I consent to the content being publicly distributed, including its upload to and display on CosplayXclusive and any other platform the Creator uses.
  4. I consent to the content being downloaded by users where the platform allows downloads.
  5. I am participating freely, without force, fraud, coercion or payment for sexual services, and I am not under the influence of any substance affecting my ability to consent.
  6. I understand I may withdraw consent for future distribution by notifying the Creator in writing, and that I can request removal from CosplayXclusive at any time under its Content Removal & Appeals Policy.
  7. The information I have provided is accurate, and I understand that falsifying it may have civil and criminal consequences.
